Please do not provide sensitive medical information through this form.

Why is information used?

Information you voluntarily send through WhatsApp can be used by the practice to respond to your enquiry and discuss a possible appointment. An enquiry is not a confirmed booking. Further processing for actual treatment or administration is subject to the practice's own privacy procedures, which still require confirmation from its owner.

WhatsApp and external services

The WhatsApp and Google Maps route links take you to external services with their own privacy policies. WhatsApp is provided by Meta and Google Maps by Google. Interactive Google Maps embeds are not automatically loaded on this website.

The website is hosted on Cloudflare Pages. Technical request information may be processed to serve and secure the website. This version does not include embedded analytics, marketing trackers or advertising, and the appointment enquiry form does not store its contents in your browser. The NL/EN language preference may be remembered locally in your browser to display the selected language. No marketing cookie banner is included for this version.
